Silence: A Never Event

I continue to develop the campaign to help with the elimination fo silence in health care. I have seen first hand the impact silence has had on patient safety as well as interpersonal relationships in the health care environment.  

When the culture does not support the ability to speak up, patients as well as those providing care are at risk. I am interested in what others have done to change the culture.
